How to Register at Walkley House
Medical Centre
The practice takes patients from Walkley, Crookes, Malin Bridge,
Wisewood, Stannington, Loxley, and heading out towards Storrs.
Patients living in the Stannington Area may find it more convenient to
register at the practice’s branch surgery, Stannington Medical Centre,
Uppergate Road, Stannington, S6 5BX, Tel 0845 1247333.
To register at either of the practices, you will need to fill in your
medical card, and take it into the surgery of your choice. If you do not
have a medical card the receptionist will give you a registration form
to complete. The form or card will then be sent to Patient Services who
will make arrangements for your medical record to be transferred from
your old GP.
When you register you will be offered a new patient health check. From
1st April 2004 all new patients will be registered on a practice list
rather than on a GP's list as previously. This does not affect your
right to see your preferred doctor, and your choice of preferred
practitioner can be recorded on your medical record if required.
Please note: When
registering please bring proof of address e.g. utility bill, bank
statement, tenancy agreement and also some photographic identification
e.g. passport or driving licence.
