Change of Details

Please let us know promptly when you change your address, name or telephone number so that we can keep our medical records up to date. We have a form at Reception to help you.

Regretfully, anyone moving outside the practice area will need to re-register with another doctor in their new locality. If you have to change doctors, we will try our best to ensure that your move is as seamless as possible by giving you contact details for finding out about local doctors’ and by ensuring that you continue to be cared for until you are properly registered with your new practice.
